Simple or joint application?

Hello, I am seeking a divorce from my spouse and are a little confused about which checkbox I should check on the divorce application:

Simple: Which is divorce only.

OR

Joint

 

In my case, I am seeking a divorce and sole custody of our daughter.  My spouse refuses to sign.

If I check off simple (divorce only), it only allows me to claim for divorce and costs.  But I want sole custody of my daughter as well.

As for joint, I did research on it and it says joint means both parties agree on all the issues, which is not the case.

 

So which one is it? I am confused…
